The (Re)Connect Program is a peer-based intervention aimed at improving retention and connection to services for Ryan White clients not currently engaged in case management. Peers who have graduated or completed 50% of the CHAMP Community Health Worker (CHW) training program reach out to clients to provide education on Ryan White services, Positively You! resources, and U=U (Undetectable = Untransmittable). They are also offering referrals to connect clients back into services, including back to case management for ongoing support if needed.
We recognize your time is valuable, so please apply if you meet the following required qualifications.
• A High school diploma, GED or High School Equivalency and
• Have completed over 50% of the CHAMP CHW Training Program (may submit application at any point in program)
Other Requirements
• Within 3 months of hire, student must complete CHAMP CHW Training Program
• Within one month of hire, all employees are required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test
